Services
Registered nurses and certified home health aides provide all patient care throughout Middlesex, Somerset, Monmouth, Mercer, Union, Essex, Morris, Passaic, Warren and Bergen counties. Staff members assess and monitor the patient's health-care needs, educate patients and caregivers and collaborate with attending physicians to promote continuity of care.

Basic skilled nursing services may include:

* Urinary catheter insertion/maintenance

* Parenteral feedings and care
Admission Services
An Admissions Coordinator:
* Processes home health referrals quickly and conveniently
* Assesses each patient's needs
* Collaborates with physicians to address home care patient's needs
* Identifies patients with complex diagnoses and recognizes the need for specialty services
Additional functions of Admission Services include:
* Providing staff consultation on home health-care orders
* Delivering necessary forms to physician offices
* Providing homecare evaluations prior to hospital discharge or in a physician's office for select cases
* Providing patient summaries, including orders, medications, changes in plan of care, and progress notes
* Submitting claims directly to third-party payers as allowed under current law
Home Care Aides
Caring World home care aides are certified medical professionals. Aides ensure home care patient care is provided in the safest and most healthful environment possible. It is important patients, family members, caregivers, and physicians understand what home care aides may and may not do.
* Supervised by a Registered Nurse, the Home Health Aide assists home care patients with personal care and hygiene services (oral hygiene, bathing or linen changes)
* Provides personal grooming (hair, nail and foot care)
* Provides housekeeping services
* Provide additional personal care services when needed
* Complete grocery or other shopping
* Provide laundry service
* Complete errands
* Accompanying clients to clinics
* Companionship and more
Specialty Programs
The purpose of the programs is to enhance the capacity of the home care patients to meet their needs, provide teaching, a special service, and close monitoring of client's condition that result in high quality care and coordination between the agency, the client, client's family, and primary physician.
The following programs and activities for clients and employees are part of Caring World services:
1. After the initial assessment is completed and the plan of care for the client is developed by the visiting nurse, the initial orientation of the assigned aide follows by general and/or specific instructions for every case, during the first week of the service.
2. The HHA receives educational material and/or a brochure about the client's disease process. (Ex. CVA, DM, renal failure, and etc.)
3. The HHA is responsible for reading and understanding of that material and should contact with Agency's Clinical Director or visiting nurse for further questions if they arise.
4. The Clinical Director is responsible for providing educational brochures to the HHA, and giving appropriate quizzes that will confirm HHA's knowledge in the certain areas.
5. Each brochure covers following elements:
* General explanation of a disease
* Methods of preventions
* Coping with disease process
* Possible complications
* Specific care of needs
* Environmental risks and life-style changes
6. The achievement by the caregiver must reach following
* Knowledge about disease
* Knowledge about appropriate care
* Demonstration of correct skills
* Demonstration appropriate behavior
7. Following brochures are available for HHA
* Caring for People with Alzheimer's disease
* Care after CVA (Stroke)
* Diabetes and Home Care
* Prevention of Pressure Ulcers
8. In some cases, if it is necessary, the HHA may be asked to monitor home care patient's vital signs, such as pulse, respiratory rate, temperature, and input/output.
Charges
Payment for Caring World services can be made through Medicaid, various government programs, worker's compensation, private insurance, or private pay. For most services, there is no out-of-pocket cost to home care patients if they qualify for Medicaid or other government programs. Some insurance companies may require pre-certification and may limit the number and type of home health visits Caring World can provide. Additionally, a deductible or a co-payment for service may be required. Patients will be informed of any additional charges for services not covered under their insurance before they receive such services. Before, or at the time of admission, home care patients, guardians, caregivers, or family members will be informed about all charges and payment methods. Caring World will provide patients, or the individual responsible for payment, with an educated estimate of such possible service charges.
Records And Privacy
Caring World employees treat patients with kindness and keep all needs private. Caring World staff members keep names, medical conditions, treatments, and family information private.
Information is not released unless appropriate authorization and consent is obtained. Caring World will create a chart for each patient - containing all physician's orders, notes and pertinent medical information. Staff members use this chart to record patient treatments and medical conditions. A patient can request to see such information through an appropriate request. Only individuals with legal rights, or those with written permission by the patient, can view a patient's chart.
